The Open Public Records Act is a state law to increase the public's accessibility to view and copy government records maintained by public agencies in the state of New Jersey. The OPRA Form below is available for your convenience to complete and submit to our Business Administrator.

OPRA Request Form
All flyers that nonprofit organizations would like to distribute to the students must be reviewed and approved by the Superintendent. Approved flyers will be posted on this website. Hard copies will not be distributed. Please complete the Request to Publicize Activities Form and send it via email to Lisa Dougherty, Secretary to the Superintendent.
The Board, fully recognizing that school facilities are the property of the community, shall make them available to organizations outside the school for educational and civic purposes only. It shall not permit the use of school facilities for individual, partnership, or corporate profit at the taxpayers expense. Nor will the Board condone any activity which will interfere with, or be detrimental to, the educational program.
The Board will permit the use of the school facilities when such permission has been requested in writing and has been approved by the School Business Administrator/Board Secretary except that the Board reserves the right to withdraw permission after it has been granted.

To improve and protect the health of Sparta Township School District students, the Health Services Department exists to provide for the well-being of students so they may further their education. To do so requires the authorization and consent of the parent/guardian, and in certain circumstances a physician, as well. This page contains the forms that your child may need in order to receive certain services while in school. Please read and complete the forms as required.
Attention Sparta High School's Incoming Freshman (Class of 2027) and Returning Athletes:
All SHS athletes participating in a 2023-2024 Spring sport MUST have their physical forms into the Nurse's Office NO LATER than February 1, 2024. This paperwork should not be submitted to the Athletic Department or the Athletic Trainers. Your forms can be dropped off in the black locked box inside the vestibule at the high school. *NOTE: Forms submitted after that date may cause a delay in the athlete's participation in practices.
The packet consists of 4 total pages 1 & 2 (Health History) to be completed by parent/guardians and reviewed by the doctor, and pages 3 & 4 (Physical Examination Form) to be completed and signed off by your doctor. The state now requires that all examining physicians have completed the Student-Athlete Cardiac Assessment Professional Development Module. All physicals are valid for 365 days. However, any athlete participating in future seasons must complete a "Health History Questionnaire".
Attention Sparta Middle School's Incoming Class of 2030 and Returning Athletes:
All SMS athletes participating in Track during the Spring of 2024, MUST have their physical forms into the Nurse's Office NO LATER than February 1, 2024. *NOTE: Forms submitted after that date may cause a delay in the athlete's participation in practices.

***NO PAPERWORK WILL BE ACCEPTED VIA EMAIL. PAPERWORK MUST BE ORIGINAL AND SUBMITTED TO THE OFFICES OF THE HIGH SCHOOL OR MIDDLE SCHOOL.
Fundraising is an important aspect of many of our sports, clubs, and activities. It is defined as any time monies or items are collected, whether it is for an event, a donation/direct appeal or to purchase a product. The school district must oversee school-sponsored fundraising events and all funds raised must be collected and processed through the school where the fundraiser is being held. All fundraising activities must be approved before requesting purchase orders or scheduling/advertising the fundraising activity. This pertains to all fundraisers (large or small) even if it is done every year.
The Fundraising Form must be completed for fundraising activities two weeks in advance of the fundraiser date. Please submit (email) the form and any supporting documents to the appropriate school's Main Office secretaries for AES, MAS, HMS, and SMS. For SHS, please email the form and any supporting documents to paula.sclafani@sparta.org.
